Based in Erina, we offer a range of programs and services across the Central Coast.

About the role

The purpose of the role is to provide Coordination of Supports and Recovery Coach services for people with NDIS packages and charges for the hours of support provided, assessing the supports and services required by consumers, their families, and their carers on an individual basis.

Key responsibilities will include:

  • Coordinate all supports for NDIS participants including mainstream, informal and formal support.
  • Coach participants through individual recovery plan development and implementation, assisting to build skills for self-direction and take control of their own lives
  • Provide and facilitate referral pathways as required
  • Support implementation of services that best suit the participant’s needs and wants
  • Engage with NDIA regional contacts, including Local Area Coordinators
  • Maintain positive working relationships and linkages with community agencies, clinical services, and all other relevant providers to ensure effective communication, referrals and support coordination can occur
  • Monitor and report progress against NDIS Goal Plans, budgets, and data collection
  • Ensure all NDIS participants have signed service agreements for all supports
  • Complete risk assessment for participants
  • Meet targets as determined for effective service provision and adhere to KPI’s as set out in Coordinator Dashboard
  • Provide and maintain quality service and portray a professional image on behalf of CCPC at all times

Key Selection Criteria

  • Relevant tertiary qualification, e.g. Certificate IV in Mental Health, Community Services, Health, Disabilities or equivalent
  • Minimum two years’ experience supporting people who live with a mental Illness.
  • Strong understanding of NDIS support items under the NDIS Guidelines
  • Experience in provision of support and services to NDIS clients
  • Demonstrated care coordination skills and a high level of discretion and sensitivity when dealing with confidential matters
  • Exceptional organisational ability to manage multiple tasks and conflicting deadlines
  • Strong working knowledge of MS Word, PowerPoint, Excel & Outlook
  • Ability to work independently with minimal supervision
  • Ability to work closely and collaboratively and build rapport with key stakeholders
  • A valid Working with Children Check for paid employment to be obtained and maintained at your own expense
  • Available to work flexible hours
  • Eligibility to work in Australia on a permanent basis
  • Driver’s license and access to a reliable and insured motor vehicle
  • A valid NDIS Worker Check (NDISWC) to be obtained and maintained at your own expense through Service NSW.

Note: Due to NDIS requirements, we are unable to employ for this position without an NDISWC. We therefore strongly encourage applicants to commence the NDISWC application prior to interviews as clearance can take up to six weeks.

Successful applicants will be required to undergo a Criminal Record Check that includes working with vulnerable groups. Police checks issued within the last 12 months will be accepted.
