About the role

This position engages and develops relationships with Aboriginal and Torres Strait Islander people with complex physical health and social support needs who face significant barriers to accessing services. This includes engagement, intake, assessment, interim response and service linkages, case management, care planning with tailored psychosocial and health supports to individuals and discharge planning both through site based and assertive outreach activities.

Your duties include:

  • Participate in outreach activities
  • Maintain knowledge, skills, qualifications, accreditations and registrations through participation in professional development activities
  • Work collaboratively with clients and the AOD and Homelessness Outreach team to conduct comprehensive assessments, identifying needs, aspirations and goals of clients
  • Link individuals into appropriate ongoing community services, including culturally specific, mental health, social support and health services
  • Assist in the supervision and induction of students, junior or other team members
  • Provide outreach support and access to essential requirements such as suitable food, medication, hygiene products, cleaning products and Personal Protective Equipment (PPE) to individuals

Our ideal candidate:

  • Must be of Aboriginal or Torres Strait Islander descent.
  • A relevant tertiary qualification such as Aboriginal & Torres Strait Islander health worker, community development, health promotion, Aboriginal and Torres Strait Islander Liaison Officer, Social Work or equivalent experience in the housing, mental health, AOD, disability or welfare sectors, a lived experience is highly valued.
  • Proof of qualification to be sighted.
  • Demonstrated knowledge of Aboriginal health and culture and ability to communicate effectively with the client group, experience in working with clients who have a history of homelessness and multiple and complex needs, as well as a sound understanding of the key issues affecting people experiencing multiple and compounding disadvantage.

All appointments are made subject to a satisfactory Police Check, a Working With Children Check (WWCC), complete a Statutory Declaration, evidence of legal rights to work in Australia, and evidence of vaccination against COVID-19 (3 doses). Additionally, some roles will also require current AHPRA registration and/or NDIS Worker Screening Clearance.

Roles that are determined to be Risk Category A or B [staff that have direct and/or indirect contact with body substances and are at higher risk of occupational exposure to infections] will be required to provide evidence of immunisation assessment including Influenza prior to commencement [A] or within 12 weeks of commencement [B]. This is in accordance with the Victorian Minister of Health Mandatory Vaccination Orders for COVID-19 and legislated requirement for influenza vaccination.
