About the role

The Homeless & Community Support Worker is a diverse role that aims to provide support to people who are facing issues of homelessness including those at risk of homelessness and/or living in inappropriate accommodation such as rooming houses and caravan parks. The position also provides support to people facing significant barriers to accessing services so that they feel confident in and supported to navigate the service system.

Your duties include:

  • Establish and maintain links with other key local agencies, local community and provide secondary consultations as required
  • Develop and implement client-directed, strengths based individual client care plans and service packages
  • Work collaboratively with client to conduct comprehensive assessments, identifying needs, aspirations, and goals of clients
  • Link individuals into appropriate ongoing community services, including mental health services, social support services and health services

Our ideal candidate

  • A relevant tertiary qualification such as Social Work or equivalent experience in the housing, mental health, disability, or welfare sectors
  • Proof of qualification to be sighted
  • Demonstrated experience in working with clients who have a history of homelessness and multiple and complex needs
  • Full Victorian Driver's License required

All appointments are made subject to a satisfactory Police Check, a Working with Children Check (WWCC), complete a Pre-employment declaration form, evidence of legal rights to work in Australia). Additionally, some roles will also require current AHPRA registration and/or NDIS Worker Screening Clearance.

Roles that are determined to be Risk Category A or B [staff that have direct and/or indirect contact with body substances and are at higher risk of occupational exposure to infections] will be required to provide evidence of immunisation assessment including Influenza prior to commencement [A] or within 12 weeks of commencement [B]. This is in accordance with the Victorian Minister of Health Mandatory Vaccination Orders for COVID-19 and legislated requirement for influenza vaccination.
