Connecting Home is a service for the Stolen Generations working across South East Australia. Connecting Home works using different programs to support survivors on their journey of healing through walking with individuals and families on this journey and using goal setting principles. At Connecting Home we believe that everyone is unique and will require tailored supports and programs to assist with an individual’s journey of healing.

ABOUT THE NDIS SERVICES OFFERED BY CONNECTING HOME

Following the implementation of the National Disability Insurance Scheme (NDIS) in 2013, Connecting Home has become a registered Service Provider to support Stolen Generations and their families with a disability. Connecting Home’s NDIS Service provides Support through Support Coordination, Plan Management and Direct Support. The Direct Support Service includes support such as community and capacity building, support with connection to culture, assisting with managing and supporting independent living and participation in the community. Whilst acknowledging the person’s Aboriginal Identity is critical, it is also essential that all aspects of a clients heritage and identity needs are acknowledged and supported respectfully.

Direct SUPPORT WORKER - NDUS (casual)

The primary responsibility of this role is to provide person centered and culturally appropriate support to our clients through the provision of personal carer, home support or Community and Capacity building.  

POSITION SPECIFIC RESPONSIBILITIES

Community and Capacity Building Responsibilities:

  • This service involves providing companionship either within the clients own home or while accessing the community to assist the client to meet their social needs. This may include shopping, social outings, attending therapy appointments, playing games and craft activities etc. 
  • To provide care assistance to each client in accordance with the service plan and as directed by the Team Leader - Support Program worker or the Team Leader – Support Coordination. This may include home care, personal care or Community and Capacity Building. .
  • To monitor the safety and well being of the client and report any concerns to the area service centre.
  • To participate in training programs as required.
  • To ensure that time sheets are submitted to the office on time.
  • To record each visit and relevant information in the communication book, where applicable.
  • To know and adhere to company politicise and practices.
  • To work in a safe manner and follow all workplace health and safety policies and procedures.
  • To report any health and safety hazards, incidents or near misses using the appropriate procedures
  • Create and maintain an environment that empowers people from the Stolen Generations
  • Maintain the highest levels of confidentiality
  • Developing positive and co-operative internal and external relationships eg, external authorities and auditors and other stakeholders
  • Other duties as directed

Home Care Related Responsibilities (may include in addition to above responsibilities):

  • Cleaning floors - vacuuming, sweeping, mopping
  • Cleaning bath, shower, vanity basin, toilet
  • Cleaning kitchen - wiping benches, cupboard doors, cleaning sink, washing and drying of dishes, emptying/loading dishwasher, cleaning stove top/oven fridge
  • Laundry - washing personal clothing and linen/towels and hanging out clothes or in dryer, removal of dry clothes from line or dryer, folding and storing, ironing.
  • Dusting and tidying
  • Making beds, changing linen
  • Meals preparation - cooking or preparation of meals or heating of frozen meals or meals on wheels
  • Shopping on behalf of client, making lists, unpacking and storing bought items
  • Errands such as banking, paying accounts.

Personal Care Responsibilities (may Include in addition to above reesponsibilities):

  • Assisting client with all aspects of daily living, this may involve full or partial assistance or supervision only. It may include any or all of the following:
  • Personal hygiene and grooming - assisting client to wash, shower or bathe encouraging client to be be as independent as possible. Assisting with dressing/undressing, hair routine and dental care.
  • Toileting - assisting client on and off toilet/comode chair, assisting with hygienic if required. Some clients may require assistance with removal and replacement of catheter bags or incontinence aids.
  • Medication - supervision and prompting of client to take medication as, and only if directed by the care plan
  • Outings with client - transport and assist client in activities such as shopping, attending appointments, social and recreation pursuits.
  • Assistance with mobility - this may range from supervision and minimal support with mobility to assistance with fitting artificial limbs or manual handling of clients, including transfers using hoists or other manual handling aids.
  • Assistance with eating and drinking, as required and monitoring of nutrition and hydration.
  • Companionship and general monitoring of client.
  • Assistance with prescribed exercises, following specific instructions/training.
  • Personal care assignments may also include duties duties listed under home care/housekeeping 

PROFESSIONAL EXPERIENCE, AND KEY KNOWLEDGE AREAS

  • Demonstrated knowledge and understanding of Indigenous societies, cultures and issues affecting the Stolen Generations
  • Demonstrated experience in communicating effectively with Indigenous peoples and communities
  • Have a good understanding of the needs and issues of people from the Stolen Generations and knowledge of the range specialist service options available from both Aboriginal and Mainstream agencies
  • Build and maintain relationships with service providers to support people from the Stolen Generations in accessing a range of generic and specialist services.
  • Well-developed interpersonal skills with the capacity to liaise effectively with a wide range of clients and service providers, including the ability to work cooperatively as a member of a team.
