ABOUT DEADLY CONNECTIONS

Deadly Connections is an Aboriginal Community Controlled, not for profit organisation. Its purpose is to build stronger and safer families and communities by driving individual, social and systemic change through a multi layered approach of providing prevention, intervention and diversionary solutions, systemic/individual advocacy and raising community awareness through education.

POSITION OBJECTIVES

Within a team approach and under supervision of the CEO, the Wellbeing Facilitator will:

  • Provide grassroots support, information, referral and advocacy to families who are participating in our programs/service
  • Receive, facilitate, and process referrals from stakeholders to our programs and to our partner agencies.
  • Work within the decolonising model and abide by relevant policy practice guidelines.
  • You will be expected to work within a collaborative team of stakeholders, including DoCaJ, DCJ, health services, other Aboriginal and non-Aboriginal agencies to promote best outcomes for participants, their families, and communities.

KEY RESPONSIBILITIES

  • Work collaboratively with participants, families, and communities.
  • Program development and facilitation.
  • Provide individualised and tailored case management support in line with our programs and their specific needs.
  • Develop care plans according to clients’ needs utilising strengths-based and decolonising practice.
  • Work with internal and external staff & other Aboriginal services for all Aboriginal families regarding cultural connection and identity.
  • To work towards strengthening family/community connection and provide early intervention and child protection/justice involvement prevention.
  • To actively participate in team work in a manner that ensures a coordinated approach to services for participants and their families.
  • Working across other support services programs as required providing support at times of crisis and when required
  • Disrupt and minimise systems involvement for mob
  • To undertake the responsibilities associated with intake and response, such as:
    • Making referral/s, completing timely and relevant follow up.
    • To undertake preliminary assessments that identify the health, wellbeing and safety needs of the individuals involved, and that identify both the capacity and constraints of families to make necessary changes.
    • Coordinating an appropriate response for program participants.
    • Entering data into the electronic system and creating paper files.
    • Ongoing communication with key stakeholders.
    • To work collaboratively with both internal and external partners and stakeholders.
    • Liaise with internal and external programs in relation to capacity, referrals and other program information.
