Directions Health Services’ multidisciplinary and innovative teams currently include a Psychiatrist, Psychologist, Social Workers, Counsellors, Case Managers, Aboriginal Health & Wellbeing Workers, Peer Support Workers, GPs, Registered Nurses, Pharmacists and Chemical Analysts.

About the position

As a Case Manager at Arcadia House rehabilitation service, you will work collaboratively with people with complex and co morbid issues, to create comprehensive treatment support and exit plans. You will facilitate groups and provide crisis and other clinical support to clients according to Therapeutic Community and Harm Minimisation principles. Working with our multidisciplinary teams, you will coordinate clients’ care across Directions Health Services’ programs.

This is a fixed term, part-time position - 0.8 FTE. Arcadia house is a 24-hour facility, and all staff are required to work after hours, evenings or night shifts when necessary.

Responsibilities

  • Provide case management, treatment interventions and group facilitation utilising a non-judgemental, person-centred, culturally sensitive, trauma informed approach, within a Therapeutic Community setting
  • Provide accurate and timely referrals to assist clients with their AOD use, other co morbidities and associated issues
  • Assist clients to identify their goals and work with them to develop a treatment plan addressing their identified issues
  • Maintain up to date and accurate case notes, data collection and other support letters and reports
  • Collect and maintain data and documentation in an accurate manner
  • Promote continuous quality improvement, participate in case reviews and work in accordance with health and safety guidelines and Directions Health Services’ Policies and Procedures
  • Work collaboratively as part of a team to facilitate day to day operations of the program in line with Therapeutic Community principles

What we would like you to have

  • A qualification or currently studying towards a qualification, in Counselling/Social Work/Psychology/Nursing/other relevant field
  • Completed a Cert IV in AOD or the 4 core competencies of the Cert IV
  • Experience working in the Alcohol and Other Drug sector
  • Excellent communication, teamwork and interpersonal skills, combined with a proactive and hands-on attitude to work
  • A good understanding of the issues our client group may experience
  • A thorough understanding of the principles of Harm Minimisation and Therapeutic Communities
  • A thorough understanding of the importance of personal and professional boundaries and ethical behaviour.

Applicants must possess a current Driver’s License and Australian Citizenship or suitable rights to work in Australia. All appointments will be conditional upon having a Working with Children Check, current Senior First Aid and CPR certifications and a satisfactory National Police Check.
