Job Description

  • Located in Ferntree Gully & Ringwood East (travel required)
  • Part-time 0.8 FTE until June 2025
  • $57.51 per hour + Super

What You’ll Be Doing

The Diabetes Nurse Educator will work alongside the Health Coach to support clients referred to the Diabetes Connect Program to address opportunities for health improvement and support them to actively engage in the management of their condition.

You will undertake assessments, planning and implementation of appropriate intervention strategies whilst providing evidence-based Diabetes Education and enhancing effective diabetes self-management.

The Team

The Chronic Disease Management Services are an integral part of EACH's Clinical and Complex Care Services, located within the Outer East Primary Health Area. Our dedicated team offers a comprehensive range of individualised and group-based services tailored to meet the diverse needs of adults with chronic and complex health conditions, supported by various funding streams.

Qualifications

Your Qualifications

  • Bachelor of Nursing, or equivalent.
  • Current registration to practice with AHPRA.
  • Graduate Certificate in Diabetes Education
  • Member of Australian Diabetes Educators Association (ADEA).
  • Credentialled with Australian Diabetes Educators Association (ADEA) or working towards this requirement (desirable).
  • A cleared National Worker Screening Check prior to commencement of employment.
  • Advance clinical skills in managing a complex caseload using client-centred approach.
  • Current Victoria Drivers Licence.
