Are you passionate about making a positive impact on the lives of individuals with disabilities? EACH is seeking a dedicated Support Coordinator to provide professional, timely, and tailored services to our diverse range of customers. Your role will involve empowering customers, families, and carers to make informed decisions, ensuring they receive the care and support they need to thrive.

As NDIS Support Coordinator, your role will involve:

  • Assisting NDIS participants with complex needs to initiate and coordinate their NDIS plan, negotiate appropriate support and services and develop their skills and confidence to manage their plan independently.
  • Working with potential customers and new customers to act as the key liaison point for all related services chosen by the individual.
  • Working alongside and connecting with community support and mainstream services, including Mental Health and Disability-related services and hospitals.
  • Ensuring all services are booked and rostered for customers in a timely way.

Your day will include actively managing a client caseload, attending meetings (office-based, community-based, and home visits), administrative tasks associated with coordinating services and report writing.

About you

To succeed in this role, you will have:

  • A relevant diploma or higher tertiary qualifications in a health or human service-related discipline and/or equivalent.
  • Demonstrated experience in community participation and service coordination that has improved the lives of people with a disability.
  • A strong understanding of managing services within a NDIS plan.
  • Demonstrated experience working in clinical, educational or community-based disability, mental health or early childhood intervention services or related fields.
  • Experience in dual-disability and/or Lived experience with a disability or experience caring for someone with a disability will be highly regarded.

Knowledge of the NDIS system and processes, excellent verbal and written communication skills, teamwork, high-level organisational skills and a demonstrated ability to utilise person-centred practices will assist you to excel in this role.
