Hope Street Youth and Family Services Limited ("Hope Street") has over 40 years of experience delivering specialist services to young people and their families experiencing, or at risk of, homelessness.

The BOOST Program provides case management to young people aged 16-25 who are homeless or at risk of homelessness and who have complex needs, dual diagnosis and challenging behaviour/s. The target group includes young people who are parenting and/or expecting birth.
The BOOST Program provides services to young people in crisis, short stay 7 night bed, supports young people in outreach (up to 3 months of service) and operates on weekdays, weeknights and Saturdays 9-5 pm, so that young people can be offered an intensive and responsive service.
We are looking for a dedicated Youth Specialist for a permanent part-time position working a regular fortnightly work pattern in Brunswick West. In this role you will provide specialist case management support to young people and their families, within a holistic client focused approach that includes housing, health, employment, education and community participation.
You will proactively engage, motivate and support the young person to identify and work towards achieving their goals. Your relationships with key support services will enhance young people’s connections to those services. You will maintain accurate client records compliant with DFFH requirements.
