THE YOUTH SUPPORT SERVICE (YSS)

The YSS Program provides case management to young people aged 16-25 who are homeless or at risk of homelessness and who have complex needs, dual diagnosis and challenging behaviour/s. The target group includes young people who are parenting and/or expecting birth.

ABOUT THE OPPORTUNITY

We are looking for a dedicated Youth Case Management Specialist for a permanent part-time position working a regular fortnightly work pattern, in Brunswick West. In this role you will provide case management support to young people and their families, within a holistic client focused approach that includes housing, health, employment, education and community participation. You will proactively engage, motivate and support the young person to identify and work towards achieving their goals. Your relationships with key support services will enhance young people’s connections to those services. You will maintain accurate client records compliant with DHHS requirements.

ABOUT YOU

  • A bachelor’s degree in Social work, Youth Work, Social Sciences or relevant Health Sciences degree.
  • Demonstrated experience and highly engaging to respond to young people with needs via responsive outreach model, and case management framework.
  • Strong experience in Case Management, Child, Youth and Family Services.
  • Extensive experience of current theoretical approaches and frameworks for working with highly vulnerable young people and their families, including strengths-based approaches.
  • Strong ability to capacity build with diverse and inclusive community services e.g. LGBTQI, Drug & Alcohol, Policing Units (proactive units) Legal services, Indigenous Services.
  • Strong ability to engage, motivate and support young people with complex needs within a case management framework and client-centred strengths-based, trauma-informed practice.
  • Strong relationship building skills which enables you to motivate young clients to achieve meaningful goals within a case management framework.
  • Highly developed communication skills including report writing, listening, negotiation, problem solving, decision making and advocacy.
  • Strong competency using Microsoft and Case management applications
