The YSS Program provides case management to young people aged 16-25 who are homeless or at risk of homelessness and who have complex needs, dual diagnosis and challenging behaviour/s. The target group includes young people who are parenting and/or expecting birth.
We are looking for a dedicated Youth Case Management Specialist for a permanent part-time position working a regular fortnightly work pattern, in Brunswick West.
In this role you will provide case management support to young people and their families, within a holistic client focused approach that includes housing, health, employment, education and community participation. You will proactively engage, motivate and support the young person to identify and work towards achieving their goals.
Your relationships with key support services will enhance young people’s connections to those services. You will maintain accurate client records compliant with DHHS requirements.
