As a NDIS support coordinator at Huntington’s Victoria you will have the privilege of working with a community living with the impacts of Huntington’s disease. In the role of support coordinator, you will directly provide assistance to the client and family in meeting immediate and ongoing support needs through provision of a range of intervention options to ensure that the client's independence, empowerment, autonomy and dignity is supported.

Please address the following key selection criteria listed below:

1. A relevant qualification in social work, community services or allied health
2. Extensive high-level experience working with people with complex needs, including people with mental health diagnoses, disability, drug and alcohol use, homelessness etc.
3. A strong understanding of the NDIS and mainstream service systems and barriers to accessing services for people with complex needs.
4. Demonstrated ability to work with a range of stakeholders and maintain collaborative client and network relationships
5. Demonstrated capacity to write comprehensive reports and psychosocial assessments
6. At least 12 months experience as an NDIS support coordinator
7. To provide strengths based, goal directed support to build resilience and improve outcomes for people with complex needs
8. Identify service system gaps and work with the care team to support access to services by providing high level advocacy
9. To facilitate challenging conversations with the client, family and/or key stakeholders to meet the needs of the client and resolve any points of conflict
10. Respond to critical incidents, and lead the care team in the development of risk management plans and communication protocols
11. Demonstrated ability to work autonomously and as a part of a collaborative team
