We are seeking a motivated Senior Wellbeing Care Coordinator to join our dedicated multi-disciplinary adult Allied Health and Nursing team. Wellbeing Care Coordinators have a pivotal role, providing client-centred assessment, referral coordination, and education to enhance self-management of chronic diseases, especially type 2 diabetes, respiratory, and cardiac conditions. Following client centred assessment and intervention, our aim is to enhance clients' quality of life and reduce avoidable hospital admissions by facilitating timely access to clinical and social services.
As the Senior Chronic Disease Wellbeing Care Coordinator, you'll collaborate with Service Connection, General Practice, Medical Specialists, Credentialled Diabetes Nurse Educators, Allied Health, Mental Health professionals, and Social Prescribing services both internally and externally. Your role involves providing clinical guidance and supervision to internal chronic disease health coaches, and partnership with program managers to ensure successful program implementation, execution, data collection, and evaluation. Maintaining detailed records of your work is essential, including statistical data.
