Impact you will have in this role

As the Senior Support Coordinator, you will have the opportunity to significantly impact the lives of consumers across Sydney by ensuring that everyone receives tailored support, directly advancing KCS’s mission to deliver outstanding support.

In this role you will be responsible for developing and refining care plans, improving the quality of life for consumers and ensuring that services are effectively tailored to evolving needs. Your knowledge and experience working with consumers living with dementia is critical to your success, as a part of your role will be implementing strategies to support the consumer and the staff in completing activities across daily living.

Your contributions to program development and adherence to high standards will help Kogarah Community Services maintain its leadership in providing exceptional care and making a meaningful difference in the community.

Key Responsibilities

  • Lead the delivery of person-centred, enablement-focused care aligned with Aged Care Quality Standards and KCS policies
  • Manage a diverse caseload, ensuring tailored care plans, regular assessments and support that meets their needs
  • Build strong partnerships with consumers, families and health professionals to provide seamless care
  • Oversee consumer budgets, ensuring cost-effective services within HCP funding allocations
  • Actively network and promote KCS in the community, increasing consumer engagement and program awareness
  • Contribute to program improvements by identifying trends and supporting proposal development
  • Ensure compliance with aged care legislation, safety standards and funding guidelines
  • Maintain accurate records and ensure timely data entry to meet legal and program standards
  • Promote KCS values by fostering cultural competency, teamwork, and professionalism in the Aged Care team
  • Act as an integral advocate for exceptional clinical care and share vital information, updates and insights as a member of the internal clinical care committee
  • Represent KCS at community events and forums, enhancing visibility and supporting social inclusion

Skills and Experience

  • Case management and or coordination experience, with relevant qualifications and experience across the Aged Care, Disability or Community Services sector
  • Experience working with consumers living with dementia is highly desirable, or alternatively knowledge and experience working with consumers with key chronic conditions such as Parkinson's, Diabetes, and Arthritis is essential
  • Proven experience in coordinating Home Care Packages (HCP) and understanding of the Commonwealth Home Support Programme (CHSP) is desirable
  • Understanding of incident management processes and the risks associated with the provision of clinical care for consumers and the organisation
  • A strong team player, who is collaborative and driven to make an impact
  • Strong ability to build and maintain partnerships with stakeholders to increase consumer interest and caseload
  • Demonstrated autonomy and initiative in managing daily tasks and competing priorities
  • Highly developed communication skills, with the ability to negotiate and develop networks with both internal and external parties
  • Advanced computer skills, particularly in Microsoft Office and consumer service management systems
  • Strong knowledge of Aged Care Quality Standards, including WHS and risk management practices
  • Experience working with diverse communities, with a high degree of cultural competency
  • Current Criminal Record Check, Working with Children Check, Driver’s Licence, and willingness to obtain a First Aid Certificate before employment
