Join our multidisciplinary team at Better Health Network (BHN) as a Care Advisor for Home Care Packages (HCP).

Our team is dedicated to delivering high-quality healthcare and support services to older adults, ensuring that they live independently in their own homes.

The team has worked collaboratively for several years, creating a supportive environment focused on client outcomes and ongoing improvement in care.

Key Responsibilities

  • Develop, implement, and review care plans that meet clients' needs using a Consumer Directed Care (CDC) approach
  • Organise and monitor brokered services with external providers
  • Manage HCP budgets, ensuring alignment with clients’ goals and current government guidelines
  • Complete required documentation for new referrals and manage ongoing care for existing clients
  • Maintain accurate client records, budgets, and expenditure reports

About you

  • Qualifications in Nursing, Social Work, Allied Health, or a related field (preferred but not essential)
  • Experience in Aged Care or NDIS Case Management
  • Knowledge of Consumer Directed Care (CDC) and Aged Care Sector Reforms
  • Strong decision-making, problem-solving, and relationship-building skills
  • Proficiency in relevant computer systems (e-Tools, PRODA, My Aged Care) and attention to detail.
