The Living Well Clinician provides direct care and assists in improving the health outcomes and quality of life for people living with chronic conditions. The clinician is involved in assessing client health needs, facilitating referrals, providing health and wellbeing information, supporting and developing self-management plans alongside clients with chronic conditions to meet short term and long-term goals. Clinical techniques such as health coaching principles and motivational interviewing are used to support the client and explore behaviour change. Interventions can be conducted in a group setting, individually or via telehealth. The clinician can also provide joint and secondary consultations with internal or external providers. The Living Well Program is made up of different health professionals that are a part of a multidisciplinary team.
This opportunity is a 0.4 Fixed term Part time position. Pending your qualification, the role is Classified under the NURSES AND MIDWIVES (VICTORIAN PUBLIC HEALTH SECTOR) (SINGLE INTEREST EMPLOYERS) ENTERPRISE AGREEMENT 2024 – 2028 - Grade CN4 at $54.35 per hour or Victorian Stand Alone Community Health Centres, Allied Health Professionals Enterprise Agreement 2022-2026 at $54.05 per hour (Grade 2 Year 4) and the Dietitians Award 2023-2026 (Grade 2 Year 4) at $53.06 per hour plus 11.5 % super.
By acting as a change champion, you will be able to support health professionals within the organisation in implementing self-management approaches with clients.
By facilitating early intervention of the client’s chronic disease and support self-management plans, the client can work towards achieving better health and wellbeing outcomes for themselves.
