The Opportunity:

  • Be involved in the establishment of the new “Locals” – key mental health system transformation.
  • Provide positive person centred and wellbeing supports for people experiencing mental ill health and emotional distress.
  • Link people to social and community services to improve health and wellbeing.
  • Be a part of a dynamic multi-agency and multi-disciplinary team delivering integrated service responses.

About the Service

The Adult and Older Adult Mental Health and Wellbeing services, otherwise known as “Locals”, are a key recommendation from Victoria’s Royal Commission into the mental health service system. As such, the Locals are a key feature of the current system transformation.

The Locals will act as a front door to the mental health service system and will provide a continuum of care and support including clinical interventions, wellbeing supports, and therapeutic support through an integrated mental health and alcohol and other drug framework approach.

Mind Australia is the lead agency in a consortium for the Melton Local which comprises of IPC Health, Western Health, and Thorne Harbour Health. Our shared vision is a Local that delivers integrated support that puts the person accessing the service at the very centre of their treatment, support and care coordination using a range of evidence informed approaches. The consortium is committed to embedding lived and living experience and expertise within all aspects of the service including program design and evaluation, leadership, service delivery and governance.

The Position

The Wellbeing Coordinators provide specialist support for older consumers, consumers with co-existing mental health and AOD issues, and consumers with a dual disability. They will have a specialist role in social prescribing and care coordination informed by trauma informed care. The Wellbeing coordinators will have relevant qualifications to assess and support people with complex needs and presentations.

You will make a difference by

  • Assessing consumer needs, providing information about the range of support options available and facilitating warm referrals to internal or external services in the “Local” area.
  • Empowering clients to recognise their strengths, aspirations and wellbeing goals and facilitate supportive connections into community-based programs that will help them achieve these.
  • Providing meaningful wellbeing coordination services that responds to target groups’ mental health and wellbeing needs through initiative-taking engagement, identifying issues impacting on mental health and wellbeing.
  • Establishing strong relationships with key internal and external stakeholders to continuously improve the social prescribing referral pathways and accessibility of community-based programs for social prescriptions.
  • Providing in person support to actively link consumers into social and community services and social groups.

To succeed, you will need.

  • Tertiary qualification and minimum 2 years’ experience working in the following fields: social work, community development, social prescribing, or psychosocial support.
  • Demonstrated experience in stakeholder engagement with proven ability to strategically engage, negotiate and influence, others to achieve better outcomes.
  • Demonstrated ability of assessment and referral facilitation
  • Excellent motivational interviewing, trauma informed and holistic wellbeing assessment skills.
  • Proven experience empowering individuals in a planned and structured way to improve mental health and wellbeing outcomes through the lens of cultural and intersectional practice approach.
