Jewish Care Support Coordination focuses on assisting participants to make choices in their life and build their capacity to take control of their future, whether that be working, participating in a program of their choice, or exploring and developing their interests. We work with NDIS participants to find the right supports for them and ensure their providers are working in a way that suits their needs.

About the Role

Jewish Care is a provider of NDIS services to people living with disability. Our services are provided within a framework that prioritises individualised active-support, assisting people with multi system need, and advocating for the rights of full inclusion, choice and control.

We are looking for a passionate, motivated, and well-organised individual to provide pre-planning, advice, and Support Coordination for people entering and sustaining services under the National Disability Insurance Scheme. You will use your exceptional interpersonal and coordination skills to support our clients to choose and control the supports they require. You will have a passion for empowering our clients in all areas of their lives, including personal decision-making, dignity and respect, relationship building, and skill development to promote full community inclusion.

Responsibilities will include:

  • Manage a caseload of clients who have contracted with Jewish Care to provide Support Coordination or Specialist Support Coordination under their NDIS plans. This includes meeting billing requirements for the service.
  • Identify and facilitate the provision of specialised and mainstream supports under the client’s government funding, including undertaking any relevant risk assessments and identifying preventative measures required to avoid crises. This includes coordinating client referrals for internal and external service provision.
  • Promote and implement all individual goal plans in accordance with the principles of inclusion and Person-Centred Active Support, including maximum physical and social integration, community inclusion, and self-direction. This includes completing relevant documentation to demonstrate outcomes.
  • Assist clients in accessing and utilising any other government and community services they may require, including providing advice, referrals, and advocacy.
  • Support and empower vulnerable people to report and speak out against instances of abuse or potential abuse.
  • Undertake regular monitoring and maintenance of service provision data, and NDIS plan utilisation data, including maintaining and updating client records as required within Jewish Care policies, funding agreements, legal, and statutory requirements.

Skills and Experience

  • Relevant qualification or equivalent experience in the disability field or a related area of expertise for example- Community Services, Psychosocial Disability.
  • Demonstrated commitment to disability and human rights.
  • The ability to deliver outstanding customer experiences, creating value through service that is customer-focused.
  • Strong planning and organisational skills, including the ability to effectively manage your own work schedule and to work both independently and as part of a team.
  • Significant exposure to the NDIS Act and the NDIA service types.
  • Demonstrated knowledge of best practices and quality standards within the wider community services sector.
  • Strong working knowledge of the Victorian Disability Act.
  • Commitment to collaborative practice, and responding to multi system need.
