The Opportunity:

As a Community Connector, you will play a crucial role in our mission to create positive change. This role is perfect for passionate individuals eager to make a real difference in the lives of people experiencing social isolation or loneliness. You will work closely with individuals and families, providing essential support and resources to help them achieve stability and thrive within their communities.

Key Responsibilities:

  • Provide comprehensive support to individuals and families, addressing their housing and community needs
  • Create capacity-building projects and programs to support priority access groups, such as new community events/support groups or community inclusion initiatives
  • Collaborate with other team members to develop and implement individualised support plans
  • Facilitate access to necessary resources and services, including housing, healthcare, and social support
  • Advocate for clients’ needs and rights within the community and with other service providers
  • Monitor and evaluate the progress of clients, adjusting support plans as necessary
  • Maintain accurate and up-to-date records of client interactions and progress

About You:

We are seeking a highly motivated person who:

  • Ability to provide client centred, holistic and strengths-based case coordination support
  • Experience in case management and the ability to plan, organise and direct resources to meet community based activities
  • Has strong interpersonal, negotiation and communication skills
  • Has the ability to work effectively both independently and as part of a team
  • Has knowledge, understanding and application of Trauma Informed Practice
  • Has the flexibility and adaptability to meet the needs of a dynamic and evolving work environment
  • Is committed to continuous learning and professional development

You Will Need To:

  • Complete our online application process
  • Hold a satisfactory Working with Children Check (WWCC) or General Probity Check must be completed, (as applicable), prior to commencing employment
  • Hold a current National Criminal History Check and maintain it thereafter
  • Hold or be willing to obtain a Child Safe Environments Certificate
  • Hold a current unencumbered Australian driver's license and be willing to drive
  • Hold or be willing to obtain a Senior First Aid certificate
  • Willing to work occasional flexible hours to meet operational requirements
