Kalwun Development Corporation Ltd is an Aboriginal & Torres Strait Islander community controlled organisation currently seeking to fill the following vacancy within our Health and Wellbeing Services division. Kalwun Health and Wellbeing Service delivers a primary health care service to the people of the Gold Coast & surrounding communities.

About the Role
We are looking for a Project Officer, Integrated Team Care, to foster collaboration and support between the mainstream primary care and the Aboriginal and Torres Strait Islander health sectors. This this is a fixed term, full time role for six months, based at the Miami clinic.
Position Duties
- Collaborating with local Indigenous health services and mainstream health services in a partnership approach for the delivery of primary care services
- Improve the capacity of mainstream primary care services to deliver culturally appropriate services to Aboriginal and Torres Strait Islander people
- Improve the integration of care across the region, including cross-sector linkages and the development of a regional plan for Aboriginal and Torres Strait Islander health for the Gold Coast region
- Increase the uptake of Aboriginal and Torres Strait Islander specific Medicare benefits schedule items, including Health Assessments for Aboriginal and Torres Strait Islander people and follow up items
- Support mainstream primary care services to encourage Aboriginal and Torres Strait Islander people to self-identify
Essential Selection Criteria
- Minimum Certificate 3 in the Health and/or Human Services sector
- Ability to facilitate training workshops
- Strong communication skills, with the ability to establish, develop and maintain relationships with key stakeholders
- Confident in the use of a range of computer software, including, but not limited to;
- MS Office Suite
- Data management systems
