We are seeking an enthusiastic and experienced individual to join a passionate team working with young people transitioning from care to adulthood.

About Better Futures:

Better Futures is a part of the broader Leaving Care response provided to young people who are or were in Out-of-Home Care funded by the Department of Families, Fairness, and Housing (DFFH).

Better Futures recognises the importance of young people having an active voice in their transition planning and provides individualised support both in-care and post-care, utilising the Advantage Thinking model across a range of life areas, including housing, health and wellbeing, education, employment, and community and cultural connections.

About the Role:

The Better Futures Case Worker will work alongside young people and their care teams early in their transition from care to identify their goals and aspirations as they move into adulthood and develop and implement detailed plans to achieve them.

Young people are provided with levels of support that flex up and down according to their needs, existing support networks and the transition goals they have identified.

The role offers support and referrals to specialist services across Advantage Thinking's five service offerings: Housing, Health and well-being, Education, Employment, and Community and Cultural Connections.

With a home base in Ringwood, the role is primarily an active outreach role across five local government areas in the Inner and Outer East of Melbourne.

About You (Key Selection Criteria):

Essential

  • A relevant tertiary qualification in Social Work, Psychology, Youth Work, Community Development and/or related fields.
  • Experienced in the case management of young people experiencing the complexities of being in and transitioning out of the Care system.
  • Demonstrated knowledge and experience in delivering trauma-informed practices that support a young person’s health and well-being through developing, implementing, and regularly reviewing individual case plans.
  • Demonstrated knowledge and experience working collaboratively with a care team to ensure a young person has a voice and agency in their transition planning.
  • Ability to work with Aboriginal and Torres Strait Islander, Culturally and Linguistically Diverse (CALD) communities, LGBQIA+ young people and their families, young parents, mental health, substance disability and other marginalised identities safely and respectfully.
  • Knowledge of and demonstrated ability to liaise with the Department of Families, Fairness and Housing, Service Australia, and other relevant government and non-government services that young people use in relation to income, physical and mental health, legal, employment, education, and community and cultural connection.
  • Ability to work independently and as a team member, contributing to continuous improvement and evaluation of the program.
  • Well-developed written and communication, negotiation, record keeping, case noting and administrative skills.

Desirable Skills, Knowledge, and Experience

  • Knowledge of housing systems including Private Rental, Social Housing and Homelessness.
  • Knowledge of vocational and non-vocational training, educational systems, and employment-related training and services.
  • Commitment to continuous development and participation in goal-directed training and professional development activities.
  • Knowledge and/or experience with delivering Home Stretch.
