We have two Case Management roles available in our Support for Families at Risk of Homelessness program (SFaR). This program delivers intensive case management services to families and children who require medium term support to successfully maintain suitable housing. The aim of the program is to tailor support so that families are prevented from entering the homelessness service system.

WHAT YOU'LL BE DOING

As Case Manager in the SFaR program, you will work with families and children for up to twelve months to deliver intensive support services based on a case-plan tailored to the needs of the family. There will be a Team Case Management approach to service delivery.

You'll be part of a program that assists families and children to positively engage in their communities of choice and, where appropriate, links families with resources in the community including education, training, health, employment and recreational opportunities. You'll focus on the needs of children and actively and positively engages children with educational, health and support services.

YOU WILL:

  • Develop case plans in conjunction with clients and implement strategies that enable clients to achieve their goals by building on their strengths and focusing on solutions.
  • Assist clients to develop longer term housing options, both within and/or independently of Launch Housing's supported accommodation program.
  • Ensure clients maximize all government and community income assistance and support.
  • Initiate and maintain contact with vulnerable clients using flexible engagement strategies to achieve case plan goals.

WHAT WE'RE LOOKING FOR (KEY SELECTION CRITERIA):

  • A valid Victorian driver’s licence (mandatory).
  • Ability to demonstrate cultural sensitivity and to adjust personal style in response to client differences.
  • High degree of interpersonal and analytical skills, including well-developed organisational and time-management skills.
  • Ability to develop and maintain positive collaborative relationships with external stakeholders to effectively enhance the support of clients.
  • Demonstrated commitment to the values of Launch Housing and our mission to end homelessness.
  • An ability to relate effectively and sensitively to families experiencing housing crisis; demonstrated understanding of the impact of homelessness on families, in particular children.
  • Demonstrated experience in client facing human services, with a preference for an understanding of housing and homelessness policy, context and systems.
  • Experience providing case management and support in an outreach capacity with an emphasis on practical assistance to maintain housing (desirable).
  • Demonstrated experience in housing or case management, or a relevant tertiary qualification.
