Launch Housing South Melbourne is a residential facility that provides temporary accommodation for families, case management support, transitional outreach support in the community and specialised services for vulnerable children and families. Launch Housing South Melbourne has 7 family units for families and children experiencing homelessness.
The Case Management team at Launch Housing South Melbourne is part of Launch Housing’s Specialist Child & Family Services area within the broader Families & New Beginnings function. The Case Management team provides flexible and responsive support to families and children to assist them in achieving dignified and sustainable housing, health and wellbeing outcomes. The Case Management team provide support to families living in the crisis accommodation units on site, as well as to families in the community via outreach services.
The Coordinator – Case Management provides effective leadership to the Case Management team at Launch Housing South Melbourne to deliver high quality, housing first and trauma-informed support services to families and children experiencing homelessness using the Launch Housing Practice Framework to guide best practice. The Coordinator is accountable to ensure that services to families and children by the Case Management team are delivered in a child-safe, consistent and professional manner with the needs and dignity of service users, including children as a priority.
The Coordinator – Case Management is also responsible for the management and administrative oversight of the 7 crisis accommodation units at Launch Housing South Melbourne and all relevant operational requirements of a Crisis Accommodation site. This includes but is not limited to vacancy management and intake and assessment of suitable families, maintenance and administrative requirements, overseeing service fees, responsive incident management and proactive risk assessment and management.
Coordinators ensure that their teams are well informed about Launch happenings, especially where it relates directly to their team’s work. Their messaging is consistent with Launch Housing’s values and supports initiatives developed by all areas of the business.
Coordinators support their staff by role modelling the Launch Housing Values, being approachable, and ensuring that every team member has a development plan and attends all relevant training for their role. They support staff performance through regular supervision and feedback, and use mistakes as opportunities to learn and grow where appropriate. They set clear and fair performance expectations for their team, including setting appropriate boundaries around client work to keep their staff safe. They know what resources are available to them and how to make the best use of them.
When leading their teams through change they communicate the vision for the change with their teams in a positive manner, listening to staff concerns and addressing them where possible. They encourage their staff to be innovative and support practice improvements through a continuous quality improvement lens.
$55.46 an hour
