About the opportunity

The Intensive Case Manager will provide an intensive level of engagement during a client’s stay at Bobs Place (Dandenong). The role will assist clients to link with internal and external services suitable to meet their individual support and accompany the client to appointments to assist with engagement and follow-up.

The Intensive Case Manager will also provide intensive outreach to their clients in the community once stable accommodation has been secured or to continue assisting with housing options for a further 6 months after exit. The support will be delivered through assertive outreach, providing continuity of care and portable support for the client across their accommodation outcomes.

This is a permanent, full-time role, 38 hours per week, based in Dandenong.

What you'll be doing:

  • Work with a multidisciplinary team to provide comprehensive intensive case management service to clients, using a strengths-based approach including assistance with accommodation, advocacy, assessment, referral and the development of community supports.
  • Conduct internal assessments to assist in measuring clients progress while participating in support.
  • Assist clients to maximise their options and potential by facilitating their involvement in community and social activities, recreational and training opportunities.
  • Advocate on behalf of clients where agreed, including liaising with mental health, drug and alcohol services, legal institutes, government and non-government service providers.
  • Ensure a collaborative service delivery by initiating/attending case management meetings with professionals and assisting clients to access specialist supports.

What we’re looking for (key selection criteria):

  • A relevant tertiary qualification in human services, and direct experience in the delivery of services to clients with complex needs and/or relevant experience.

  • Demonstrated understanding of housing and homelessness policy, context and systems.
  • Demonstrated ability to work collaboratively with others towards effective client solutions.
  • Good interpersonal and communication skills, both verbal and written.
  • Well-developed organisational and time management skills.
  • Demonstrated ability to build positive relationships and communicate with people of diverse backgrounds and abilities.
  • Excellent problem-solving skills.
  • A valid Victorian Driver’s License.
