Lighthouse Foundation provides therapeutic care programs to children and young people impacted by long-term neglect, abuse and homelessness. Our focus is to create caring communities where young people can feel safe, form meaningful relationships and begin to heal.

ABOUT THE ROLE

Lighthouse are looking for a Therapeutic Case Manager to join the Community Connect team, delivering outreach based therapeutic case management.

The role will support young people leaving out-of-home-care to transition to independent living, as well as young people experiencing or at risk of homelessness.

The role will use a case management framework to support vulnerable young people to transition into meaningful independent living. The primary duties will involve a combination of care planning and the delivery of individualised and community therapeutic interventions.

ESSENTIAL REQUIREMENTS

  • Tertiary qualification in Youth Work, Social Work or in similar discipline is preferred. Related experience highly valued.
  • Experience and knowledge of responding to young people and their unique needs
  • Experience in delivering case management
  • Strong ability to engage, motivate and support young people with complex needs using a young person centred, strengths based and trauma informed approach.
  • Great relationship building skills which enables you to meet young people’s needs and achieve meaningful goals within a case management framework.
  • Demonstrated capacity to work flexibly, and have the ability to manage competing demands.
  • Excellent communication, verbal and written skills.
  • Strong organisational time management and computer skills
