ABOUT THE ROLE

  • Providing Therapeutic Case Management for young people with experiences of homelessness
  • Supporting young people to effectively and therapeutically plan transitions to independent living and break their cycle of homelessness
  • Developing stakeholder relationships within the community and be an effective advocate for young people
  • Role modelling a warm and welcoming home environment, to form meaningful relationships
  • Managing a small caseload of up to six young people to ensure high quality case management outcomes
  • Part of a culture in which young people and staff are truly valued and community members feel a sense of belonging

This role’s purpose is to support vulnerable young people to transition into meaningful independent living. The primary duties will involve a combination of detailed transition and care planning, convening an external network of individualised service supports, delivery of therapeutic interventions and managing operations and culture in the home through Lighthouse’s Model of Care.

The Therapeutic Case Manager employs a therapeutic approach within a multidisciplinary team to support young people in their care. The role will be based from the Lighthouse Kensington home predominantly and Youth Recourse Centre Hub Location and will require evening and weekend work.

ESSENTIAL REQUIREMENTS

  • A minimum Cert IV in Child, Youth & Family Intervention or other higher level qualification in Social Work/Youth Work equivalent is essential. However, Lighthouse will employ applicants who commit to enrolling in a relevant qualification.
  • Applicants must also complete the mandatory up-skilling units as required by Department of Families, Fairness and Housing.
  • Hold a current Working with Children Check (Employee)
  • Complete a National Criminal Records check, and participate in a psycho-social screen by Lighthouse,
  • Provide consent for a Disqualified Carers Check
  • Hold a valid Victorian driver's licence
  • Qualification/training and considerable knowledge of working with young people and the impacts of youth homeless/ Mental Health
  • Experience in a case management or care orientated role
  • Demonstrated capacity to understand the needs of young people, in particular young parents and their babies
  • Well-developed risk assessment and risk management skills
  • Ability to work independently and as part of a team
  • Ability to plan, prioritise workloads, and meet deadlines as required
  • Excellent written and verbal communication skills
  • Experience with conflict resolution
  • Willingness to participate in reflective practice
