• Salary $74,179 to $88,702 + Super + full salary packaging up to $18,540 per year

About the role

As an Alcohol and Other Drugs Counsellor Case Manager, you will provide a comprehensive range of clinical and psycho-social interventions to people requiring AOD services. This role will manage a case load of clients providing individual counselling & case management, including specialist assessment on the level of drug dependence. You will thrive in this opportunity to:

• Coordinate the intake and assessment for new clients and formulation an effective treatment plan
• Manage a case load of clients, including in an outreach capacity, through providing support, service coordination, referral and advocacy to assist client in achieving their goals
• Participate in case conferencing with supervisor, clinicians, and other agencies.
• Ensure continuity of care by effectively communicating within the multidisciplinary team and across interfaces with other health care providers

About you

• You have demonstrated knowledge in AOD counselling, case management, intake & assessment
• You can work autonomously and effectively manage your own workload
• You have highly developed communication and community engagement skills
• You have proven capacity to work flexibly in a team within a community-based setting

What do you need to apply?

• Tertiary qualifications in social work, counselling, psychology, mental health, or related discipline
• Applied knowledge in contemporary evidence based AOD counselling practice
• Hold a Blue Card (or ability to apply)
• Current unrestricted Australian Drivers Licence
• Knowledge of the local Toowoomba region is desirable
