Merri Outreach Support Service (MOSS) is dedicated to empowering individuals at risk of or experiencing homelessness. As an Outreach Case Manager with the Housing Support for the Aged Program (HSAP), you'll provide vital support to older individuals experiencing or at risk of homelessness. This part-time role (24 hours per week) is based in Brunswick and focuses on assertive outreach, short-term case management, and connecting clients to essential services. You'll engage in community development activities to reduce isolation, enhance safety, and empower clients to live independently. Join a collaborative team committed to making a meaningful impact.
We are seeking a candidate with a qualification in community services, social work, or a related field, along with experience supporting vulnerable populations, especially older adults. The ideal candidate will have strong case management and advocacy skills, a solid understanding of aged care and public housing systems, and a commitment to social equity. Excellent communication, interpersonal, and organisational skills are essential, as is the ability to collaborate effectively and support diverse communities within a team environment.
