Miwatj Health is seeking for Chronic Conditions Program Lead to provide meaningful healthcare outreach across the East Arnhem region. The Chronic Conditions Program Lead will provide high quality, culturally appropriate, strategic direction in chronic disease service delivery in the Miwatj Region. This role also support and guide Miwatj Chronic Disease portfolio holders and staff working with people living with chronic conditions with a focus on the Zone 2 Clinics.

There are a range of benefits in this 12-month fixed term position, including a competitive annual salary ($131,739 - $137,268 per annum + superannuation), and a Nhulunbuy or Darwin base with travel across East Arnhem land in light aircraft.

Miwatj Health is an independent Aboriginal Community-Controlled Health Service. Established in 1992, we’re committed to securing quality health care services across the East Arnhem and enabling Yolŋu powered community health and wellbeing. Miwatj provides culturally appropriate, comprehensive primary health care to over 6,000 remote Aboriginal and Torres Strait Islander residents across 3 East Arnhem wards. With a Head Office in Nhulunbuy, Miwatj also manages a Corporate Office in Darwin, 7 fixed clinics across the East Arnhem region, and dedicated outreach teams who travel to 3 additional locations.

Essential Requirements

The Chronic Conditions Program Lead will be responsible for providing program delivery support and expertise to remote Primary Health Care (PHC) staff in the management of chronic conditions. Ideal Candidate will collaborate with internal and external stakeholders to support strategy development and to ensure a multi-disciplinary approach to the implementation of the Northern Territory (NT) Chronic Conditions Prevention and Management Strategy. This role has an essential requirements to understand and commitment to Aboriginal Community Control, self-determination, and the issues faced by Yolŋu people living in remote settings.
