The Care Finder Program is a part of consortium comprised of Micah Projects, Communify Queensland and Queensland Positive People that targets people who have one or more reason for requiring intensive support to interact with My Aged Care, access aged care services and/or other relevant supports in the community.

How you will make a Difference:

As an Aged Care Navigator, you will work as part of the Service Integration and Health Cluster to advocate for and support individuals so they are empowered to access appropriate resources, opportunities and aged care services within the community.

This is a full time, fixed term employment opportunity until 30 June 2025, with conditions and salary at Level 4/5 of the SCHADS Award dependent on qualifications, skills, and experience.

To be considered for this role, you’ll need to hold a relevant Human Services qualification and/or extensive experience, or a combination of experience, expertise, and competence.

We are looking for people who have demonstrated experience in providing a holistic, trauma-informed, participant-centered approach to support and advocacy.

Demonstrated knowledge and experience of working with people who have experienced disadvantage, engagement and interpersonal skills using a non-judgmental approach and an understanding of assertive outreach and trauma informed care is essential to this role.
