Job Description

The Care Finder Program is a part of consortium comprised of Micah Projects, Communify Queensland and Queensland Positive People that targets people who have one or more reason for requiring intensive support to interact with My Aged Care, access aged care services and/or other relevant supports in the community.

We are seeking a compassionate and dedicated Aged Care Navigator to join our Care Finder Program. This role involves providing specialist and intensive assistance to help individuals understand and access aged care services and connect with other relevant supports in the community.

This is a fixed term until 30 June 2025, full time employment opportunity with conditions and salary at Level 5 of the SCHADS Award.

Desired Skills and Experience - Key Responsibilities:

  • Promote a culture of respect, safety, and trauma-informed approaches.
  • Assist individuals in understanding My Aged Care and available support opportunities.
  • Conduct assertive outreach to engage and build rapport with potential participants.
  • Undertake holistic assessments to determine individuals’ needs and access supports.
  • Establish and maintain relationships with organisations to improve participant access to aged care and specialist services.
  • Maintain participant relationships throughout the duration of support.
  • Collaborate with other care finder organisations to ensure comprehensive support for participants.
  • Contribute to a safe workplace in compliance with the Work Health and Safety Act 2011.

Qualifications and Experience:

  • Relevant Human Services certificate, diploma, or tertiary qualification and extensive experience, or a combination of experience, expertise, and competence.
  • Demonstrated knowledge and experience working with disadvantaged individuals.
  • Understanding of assertive outreach and trauma-informed care.
  • Ability to work collaboratively within a team and with external stakeholders.
  • Effective communication skills, both written and verbal, and IT competencies.
  • Commitment to cultural diversity and knowledge of equity and WHS principles.
  • Positive attitude and ability to work flexibly and proactively.
