We are seeking a skilled individual with experience from diverse backgrounds to support our growing Support Coordination services.

About the Role:

As a Support Coordinator/Psychosocial Recovery Coach, you will play a vital role in connecting participants with the resources they need to thrive. You will work collaboratively with participants, families, and healthcare providers to develop and implement individualised support plans, ensuring that our participants receive the highest quality of care.

Requirements:

• Motivation to meet targets, work collaboratively in a team, and work in a field where there is always something new to learn
• Demonstrated experience with NDIS Support Coordination highly desirable.
• Excellent high level communication skills including verbal, written and interpersonal
• Can work creatively to find positive outcomes
• Must have current DHS Working With Children check and NDIS Worker Check
