The Way Back Support Service (TWBSS) program is a suicide after-care service that has been developed to fill an unmet need in the community and health system. It was designed to care for people discharged from hospital following a suicide attempt or crisis and support them in their recovery.
Reporting to the Team Leader, the role of the Support Coordinator is to work collaboratively with existing services (both clinical and community based), supporting participants in the first three months following a suicide attempt and help an individual self-manage distress, stay safe and connected. The successful applicant will participate in the intake and assessment processes, implementation of individualised plans and assist participants in building long term support networks in the community.
