The Way Back Support model provides support to people most at‐risk of suicide: people who have attempted suicide or those who are experiencing a suicidal crisis.
The role of a Suicide Prevention Support Coordinator will help a person stay safe and connect with existing health and community services during a period of high risk and vulnerability. The role will ensure that people are supported to stay safe, to access treatments and to connect with supports to help avoid another suicide attempt. The Support Coordinator will provide continuity of care/care coordination by engaging with individuals at risk following a suicide attempt or suicidal crisis and acting as a link between relevant supports/services. In addition, the Support Coordinator will work alongside local families and communities in delivering community awareness raising programs and supports with the aim of encouraging help seeking and reduction of stigma associated with suicide.
