About the role

We are seeking a compassionate, self-motivated Community Mental Health Practitioner eager to make a difference. This permanent, full-time opportunity in Port Lincoln allows you to support clients in achieving their recovery goals, build capacity to live safely in the community, strengthen social connections, achieve wellbeing goals and live independently.

As a Community Mental Health Practitioner, you will play a key role in empowering individuals through person-centered, recovery-oriented support. Our NAPSS ICSS Psychosocial services focus on goal setting, transition planning, as well as application and transition to NDIS services. We use a two-tiered approach:

  • Tier 1: Intensive Crisis Support

Provide flexible, intense support and case management for a limited period (6 weeks) to reduce immediate distress, enhance personal safety, and connect clients to long-term community services.

  • Tier 2: Non-acute Psychosocial Support

Offer moderate support over a four-month period (extendable to 12 months) to help clients achieve recovery goals, build daily living skills, maintain housing, and foster social connections. Assist clients with NDIS eligibility, application, and transition.

Key responsibilities

  • Deliver high quality services and facilitate positive educational and social experiences.
  • Support clients to return to their community and enjoy a meaningful life through an individualised approach to recovery.
  • Provide information and mentoring to clients to establish relationships with families and carers, create community networks and access to relevant services.
  • Plan and deliver group programs that aid clients to build their skills and focus on recovery.
  • Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to client’s recovery goals.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
  • Proven capability to implement therapeutic, recovery-oriented frameworks and practices including risk mitigation, trauma informed care and family inclusive practice.
  • Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers, clinical teams and key stakeholders.
