The Rockingham Sub-Acute, short stay Step-Up Step-Down (or Kwelena SUSD) residential service, delivered in partnership with Southern Metropolitan Health Service and Housing Choices, supports clients up to 30 days to transition from an acute hospital setting or prevent hospital admissions for people entering directly from the community. Clients are supported 24/7 to achieve their recovery goals, build daily living skills, strengthen relationships with families and carers, develop community networks, build confidence and capacity to live safely in the community. KSUSD works within a recovery focused, clinical treatment framework providing personalised 1:1 support, shared supports and group activities based on client’s strengths and goals as identified in their recovery plan.
We are seeking a compassionate, resilient Community Mental Health Practitioner for a Casual opportunity to work within a close knit team on a 24/7 rotating roster in the Rockingham area. You will provide intensive person-centred, recovery-oriented supports to clients to achieve wellbeing goals, build capacity and daily living skills, strengthen social connections, enhance community networks and grow confidence to live independently in the community.
