About the role

Our Community Care Units (CCU) provide recovery-oriented residential rehabilitation to clients who are over 18 years of age that have complex needs that require more intensive, extended, residential rehabilitation assistance and support to transition into community living. The CCU’s operate 24 hours a day across a 7 day roster.

We have a permanent, full time opportunity as a Community Mental Health Practitioner – Cultural in our Community Care residential unit in Toowoomba. Working with Aboriginal and Torres Strait Islander peoples you will deliver a high quality recovery oriented, direct psycho-social support and case work to clients, their families and carers using Mind’s Model of Recovery.

Key Responsibilities

• Provide support to clients using targeted and integrated case managed responses.
• Support clients by delivering high quality service and facilitating positive educational and social experiences.
• Operate collaboratively with a multidisciplinary team and the community to ensure a co-ordinated and integrated response to the client’s recovery goals.
• Plan and deliver group programs that aid clients to build their skills and focus on recovery.
• Provide consultation to the team and clinical partners to raise awareness of health outcomes for Aboriginal and Torres Strait Islander peoples.

What you’ll bring

• Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
• Expertise in working with Aboriginal and Torres Strait Islander peoples with mental health issues who have complex needs and experienced interacting with families and carers.
• Demonstrated understanding of the Community Services sector and ability to build strong relationships with agencies, clinical providers, local communities and elders.
• Relevant experience in facilitating group activities for clients experiencing mental ill health.
