The Fee For Service (FFS) program offers a range of specialist interventions and approaches to people with psychosocial disability and complex needs who have NDIS plans and/or TAC and WorkCover support packages with the focus on maximising recovery outcomes in a fee for service model. The FFS program provides centre based and tele-health services, outreach services, support coordination, and education and vocational support. The service offers clients choice and control over services so they are able to build life skills and capacity, develop social connections and create community linkages to identify and achieve their recovery goals.
We are seeking a self-motivated, compassionate Specialist Support Coordinator for a fixed-term, part-time (22.8 hours/week) opportunity to work alongside a dynamic team in the Melbourne metropolitan area. Working from home, you will provide Level 3 Support Coordination to assist participants to address barriers and reduce complexity in the mainstream and community supports, helping them to overcome any obstacles to the implementation of their NDIS funded supports. In this role you will apply the principles of choice and control by ensuring awareness and options of services are offered to the participants.
