The Youth Psychosocial Recovery Service (YPRS) is a psychosocial support model for young people aged 16-25 years who have a mental illness and other complexities. The service provides intensive case management underpinned by Mind’s Model of Recovery Oriented Practice to support young people pre and post entry to a Youth Residential Rehabilitation (YRR) service and as an alternative to YRR placement. YPRS will facilitate wrap around support, linkages and utilise brokerage funding to assist in responding to need.
We are seeking a resilient, compassionate Youth Outreach Recovery Support Coordinator to work in the Wondonga area. You will engage young people through assertive outreach, coordinating wrap around supports and working holistically with individuals within a recovery-oriented framework.
