About the role

The Youth Psychosocial Recovery Service (YPRS) is a psychosocial support model for young people aged 16-25 years who have a mental illness and other complexities. The service provides intensive case management underpinned by Mind’s Model of Recovery Oriented Practice to support young people pre and post entry to a Youth Residential Rehabilitation (YRR) service and as an alternative to YRR placement. YPRS will facilitate wrap around support, linkages and utilise brokerage funding to assist in responding to need.

We are seeking a resilient, compassionate Youth Outreach Recovery Support Coordinator for a permanent, part time (30.4 hours per week) opportunity to work in the Wondonga area. You will engage young people through assertive outreach, coordinating wrap around supports and working holistically with individuals within a recovery-oriented framework. 

Key responsibilities

  • Provide support to young people using intensive targeted and integrated case managed responses.
  • Deliver high quality services and facilitate positive educational and social experiences in a residential setting.
  • Plan and deliver group programs that aid clients to build their skills and focus on recovery.
  • Work as part of a multidisciplinary team offering specialised psychosocial support.
  • Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to the client’s recovery goals. 

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Expertise in working with young people with severe mental health issues, complex needs and collaborating with families and carers.
  • Experience in assessing need and working collaboratively to plan goal focused recovery using evidence informed approaches and tools.
  • Demonstrated understanding of the NDIS and Community Services sector and ability to build strong relationships with service providers, clinical teams and key stakeholders.
