Your Opportunity

This is an exciting and rewarding opportunity to support the implementation and promotion of the innovative new Reach and Refer program into selected general practice clinics across the Central and Northern Adelaide metro area.

You will provide holistic nursing care to people aged over 40 who are identified with risky alcohol consumption, in a safe and supported environment. Supports include screening, motivational interviewing, brief interventions, information provision, support planning and delivery, care coordination, symptom management, delivery of evidenced-based therapeutic interventions, and assisting the facilitation of referrals to specialist alcohol counsellors.

Your key responsibilities will be to:

  • Provide intensive ongoing person-centered case management, with complex clients and review progression against case plan
  • Address the needs of older adults (aged 40+) with risky levels of alcohol consumption who present to a general practice, through brief interventions, motivational interviewing and informational support
  • Intensive client-centered counselling will be provided, with individualised interventions tailored to each client's needs, incorporating harm reduction strategies and evidence-driven best practice
  • Provide treatment interventions in accordance with the Adelaide PHN AOD Treatment and Quality Framework, comprising: counselling, care coordination, information and education sessions
  • Build and maintain positive internal and external relationships, with the ability to adapt communication style to resolve conflict including community, government non-government services, Indigenous organisations, clients, and their family/caregivers

Requirements for Success

  • Registered Nurse with current APHRA Registration
  • Demonstrated understanding of working with people over 40 years of age with problematic alcohol issues within the context of harm reduction
  • Experience working in a mental health and/or alcohol and other drugs setting
  • Demonstrated ability to provide specialised or complex therapeutic interventions
  • Well-developed written and verbal communication skills
  • Willingness to travel to various sites across metropolitan Adelaide (pool car provided)
  • Current and valid Drivers Licence
  • Current Working With Children Check
  • Up to date COVID-19 vaccinations
