Your Opportunity

Mission Australia's Psychosocial Services in the Northern Territory help individuals with mental illness transition to the NDIS or provide support if they're ineligible. Following a recovery framework, we assist in achieving non-clinical goals for housing, finances, daily living, work, and overall well-being.

This pivotal Case Manager opportunity will see you connect with people living in identified remote communities who are living with complex mental illness. Case management is provided in person and via tele-health so you will need to have the flexibility to travel regularly to Mataranka, Jilkminggan, Barunga, Wugularr (Beswick) and Pine Creek.

Your Key Responsibilities

  • Respond to referrals of individuals from internal and external services and conduct initial assessments of suitability for support
  • Undertake formal face to face assessments of clients, including completing all necessary paperwork and data entry using the approved assessment tool
  • Engage with participants in a culturally appropriate manner, fostering connection and trust
  • Develop strong, positive relationships with key stakeholders
  • Provide intensive ongoing person-centred, culturally aware, solution focused and recovery-based case management
  • Create and maintain comprehensive individual case management files for all allocated people in line with Mission Australia protocols with regular plan reviews.
  • Complete a range of internal and external reports
  • Attend local community meetings, forums and community awareness events and conferences as required

Requirements for Success

With a significant background in Mental Health and/or Community Services sectors, you will be confident in your case management and assessment processes.  Other requirements include:

  • Minimum Certificate IV qualifications in Mental Health or equivalent with previous experience working in the mental health sector
  • Deep understanding of Aboriginal and Torres Strait Islander cultures and communities - previous experience working within remote or regional groups
  • Ability to travel to remote communities on a regular basis, involving some overnight stays
  • A sound understanding of the National Disability Insurance Scheme (NDIS) access process
  • A demonstrated person-centred, recovery-focused, strength-based approach to participants' care, based on a sound understanding of mental health and illness
  • Highly developed verbal and written communication skills
  • Experience in developing and/or facilitating one-on-one and group activities
  • A valid NT Working with Children Check (OCHRE Card)
  • Full driver licence with no restrictions

Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
