The Disability Leaving Care Advisor role will be focused on building relationships with stakeholders to prioritise leaving care planning for young people who have, or may have a disability, to ensure that these plans; are developed with the young person's care team; are comprehensive and detail the supports a young person requires to transition from care, and are aligned with the young person's goals and aspirations.
The Disability Leaving Care Advisor will provide advocacy and support to care teams to navigate the complexities of the service system so that young people with disabilities can transition from care with the supports they require.

The opportunity is a full time role on a fixed term contract for 12 months with an opportunity to extend.
Salary range (excluding Super) is negotiable between $88,761.92 and $92,773.20 depending on experience and skill set of candidates. Generous salary packaging provisions also apply.
