Due to internal promotion we are looking for an experienced and dedicated Aboriginal Case Manager to join our team at Momentum Collective. As the Aboriginal Case Manager for Homelessness Services, you will play a vital role in supporting and advocating for individuals within the community who are experiencing homelessness or at risk of homelessness. This posiiton can be based in Tweed Heads or Ballina.
- Provide culturally appropriate case management and support services to First Nations people and their and families experiencing homelessness.
- Develop strong, trusting relationships with clients to understand their needs and goals, and provide tailored support to help them achieve stability.
- Work collaboratively with internal teams, external service providers, and community organisations to facilitate access to housing, healthcare, employment opportunities, and other essential resources.
- Advocate on behalf of clients to ensure they receive fair and equitable treatment when accessing services and support.
- Conduct comprehensive assessments of clients' needs and develop individualised care plans with achievable goals that empower clients towards self-sufficiency.
- Maintain accurate and up-to-date case notes, reports, and client records in accordance with organisational guidelines.
This is a full time fixed term role to 30th June 2026 with the possibility of on-going work.

ABOUT YOU
Essential Criteria
- This position is an identified Aboriginal position and therefore Aboriginality is a requirement of the job as well as cultural association with one or more of the local Aboriginal communities.
- Cert IV Community Service and or other relevant qualification and or case work experience in an associated field.
- Current Driver’s Licence.
- Satisfactory Criminal record check.
- Current Working with Children Check (NSW).
- Evidence of full COVID19 vaccination.
- Understanding and experience of working with Aboriginal people and their communities.
- Demonstrated experience in case planning and coordination of people with complex support needs.
- Understanding of, and demonstrated ability to work with Human Service Agencies.
- Demonstrated experience and or understanding of Trauma Informed Care.
- Excellent communication skills, particularly in the areas of negotiation, facilitation, mediation and problem solving.
- Intermediate computer literacy skills including Microsoft office suite and databases.
