About The Role

The Community Rehabilitation (Home Based) program offers rehabilitation services to clients within the context of their own home and community. The case manager undertakes the comprehensive assessment, develops the care plan and works closely with the interdisciplinary rehabilitation team coordinating therapy and planning for discharge. As an integral member of the team the successful applicant will have the opportunity to be a creative thinker and deliver holistic care solutions for clients in their own homes.

This is a fixed term part time position (64 hrs / fortnight). 4 April 2022 - 23 October 2022

About You 

The successful applicant will have a professional qualification in an Allied Health discipline, have broad experience in rehabilitation settings and possess sound knowledge of community resources. High level interpersonal and problem solving skills will be required. Regular supervision is provided.

What You Need:

  • Relevant Qualification (Allied Health Professional)
  • Demonstrated experience in rehabilitation and / or community based care
  • Current AHPRA registration ( As required)
  • Current Victorian Drivers Licence
  • Australian working rights
