We are a busy and dynamic team assisting patients and families by providing a professional service for patients referred across acute medicine and outpatient clinics.

  • Our Social Work service includes acute and specialty areas such as Intensive Care, Emergency Department and Women’s and Children’s services.

  • This position is based at Moorabbin Hospital.

We are looking to appoint a Grade 2 Social Worker (0.4 EFT) to support our Chemotherapy Day Unit patients at Moorabbin Hospital. This is an onsite role, working collaboratively with the entire Chemotherapy Day Unit team and supported by the inpatient Social Work team based at Moorabbin, in Melbourne's South-Eastern suburbs. A Day in the Life

You will be part of a flexible workforce with personalised support.

In this CDU Social Work role, you will be responsible for triaging new referrals and meeting with CDU patients either in person during their treatment onsite at Moorabbin, or via Telehealth as an outpatient. The focus will be primarily conducting psychosocial assessments, risk assessments, providing emotional support and working collaboratively with relevant parties (CDU staff, Palliative Care, care providers).

What you bring:

  • You are a qualified and experienced Social Worker with a minimum of 2 years post graduate experience providing a strong focus on patient centred care
  • Through your hard working and enthusiastic nature you have the ability to adapt to changing settings and shifts during the business week and weekends or public holidays
  • You are a dynamic Allied Health professional who is passionate about making a positive contribution to the health of your patients
  • You are highly motivated to perform at your best every day, demonstrating excellent communication and interpersonal skills
  • Your dedication to safe best practice and quality care
  • An unwavering commitment to delivering patient centred care
  • Possess a positive attitude to your work, compassion and the ability to work collaboratively and effectively in a team
  • Motivation to be accountable for and manage specific portfolios in our relentless pursuit of excellence
  • Experience in a hospital or other health setting is desirable

What you need:

To be considered for this important role you will ideally possess the following:

  • An AASW accredited Bachelor degree or entry level Master’s Degree of Social Work that provides eligibility for membership of the Australian Association of Social Workers
  • Minimum of 2 years’ experience in a healthcare setting
  • Good organisational skills and a positive, enthusiastic attitude towards work
  • Experience in working with culturally and linguistically diverse communities
  • Able to work autonomously
  • A friendly, outgoing and reliable demeanour
  • Well-developed clinical assessment and complex discharge planning skills
  • A current Victorian drivers licence
