The Pregnancy Family Meetings (PFM) program utilises family meetings with an independent facilitator to promote early engagement and interagency planning with pregnant women and families at risk of their newborns entering Out of Home Care at birth. Early engagement and partnerships between Health and the Department of Communities and Justice aims to reduce risks and the number of infants assumed into care.
The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).
NBMLHD have on offer an exciting opportunity for a health professional to join the Pregnancy Family Meetings (PFM) team. The Pregnancy Family Meetings Program utilises family meetings with an independent facilitator, to promote early engagement and interagency planning with pregnant women and families at risk of their newborns entering Out of Home Care at birth. Early engagement and partnerships between Health and the Department of Communities and Justice aims to reduce risks and the number of infants assumed into care. The program utilises a strengths-based model which promotes a participatory and transparent process for parents and families.
The successful candidate will work across the Nepean Blue Mountains Local Health District (NBMLHD) and Department of Communities and Justice (DCJ) (WS&NBM District) as the primary Facilitator. Your role would see you working directly with families and agencies to schedule and then facilitate Pregnancy Family Meetings. Alongside the Pregnancy Family Meeting Coordinator you will receive and triage referrals liaising with referring DCJ Case Workers. You will build and maintain regular contact with relevant internal and external partners including, but not limited to, Department of Communities and Justice, Maternity Services, Child and Family Nursing, Perinatal and Infant Mental Health Services (PIMHS), Substance Use In Pregnancy And Parenting Service (SUPPS) and Aboriginal specific services.
Your success as Facilitator will be demonstrated by your ability to exercise professional judgment in response to complex issues when
providing consultation and advice to managers, services, clinicians, and external agencies.
