  • Full Time, Maximum Term Contract until 30th October 2025 (Parental Leave)
  • Classification: SCHADS Level 4 with a salary range of $85,205 - $91,686 per annum, pro rata + Superannuation

About your new role

Community Care Coordinator within The Way Back NSW team, providing community-based care and coordination to people following a suicide attempt or suicidal crisis.

The Community Care Coordinator will provide strengths based, consumer-led and culturally appropriate care and support to people after a suicide attempt or experience of distress/suicidal crisis.

Community Care Coordinators will provide one-on-one, flexible, psychosocial support and care coordination for approximately three months, to:

  • Develop a safety plans and multi-agency care with individuals
  • Provide one-on-one service navigation, coordination and guidance to establish and maintain clinical and social networks to support safety and recovery.
  • Establish a strong therapeutic alliance ensuring continuity of care, including engagement with the individuals GP, or in the case where there is not a regular GP, linking them with one
  • Link participants in with clinical support through The Way Back Mental Health Professional where appropriate and work in collaboration with TWB MHC to support participants to meet their goals.
  • Engage the persons support network, providing resources and culturally appropriate support where required.
  • Provide a one month follow up as required to ensure people remain linked with their appropriate health network to facilitate ongoing care

What you will bring to the role

  • Relevant qualifications (minimum Cert IV level or other relevant qualification in human services) and/or experience in mental health or suicide prevention work
  • Experience and understanding of working with people with suicidality and/or deliberate self-harm
  • Sound understanding of the personal, family, and social issues associated with suicidal ideation and attempts, including awareness of groups identified to be at increased risk of suicide
  • Experience liaising with external service providers, including specialist mental heath services, GP's and other community-based services and organisations.
  • Ability to work well autonomously and within a multidisciplinary team

  • Current Working with Children Check - required before commencement of work
