We are proud to support 32,000 people living with mental health challenges across Australia and offer more than 70 services, over 49 different Indigenous lands, across the nation. These include mental health and wellbeing supports, housing and suicide prevention services.
The Commonwealth Psychosocial Support (CPS) Service offers individual coaching and group activities to support recovery for people with severe mental health within their local communities, who are not eligible for the NDIS.
CPS supports consumers by:
The CPS Service is funded by the Northern Melbourne Primary Health Network (PHN) and covers the following regions; Darebin, Hume, Merri-bek, Yarra, Melbourne, Moonee Valley and Macedon Ranges. The CPS Service is supported by internal programs including NDIS Interface who solely supports consumers with NDIS applications, and Integrated Chronic Care (ICC) who support consumers with co-existing chronic physical health conditions.
As an organisation who understands the importance tailoring support provision to the communities it works within, Neami is seeking a CRSW or PSW who has established knowledge and connection in the Macedon Ranges area. As the Macedon CRSW/PSW in the CPS team you will provide short to medium term community based psychosocial support to a caseload within the region. You will also have a strong focus on building and maintaining community connection in this region, enhancing referral pathways for consumers living in the catchment. The Northern CPS team has ongoing partnership with a number of Community Health Organisations and the successful candidate will work together with these services in providing holistic support to consumers in the area while identifying other relevant partners and networks in the area?
Community Rehabilitation and Support Workers provide a range of support to consumers, tailored to meet their individual needs. Neami National's work with consumers occurs within their own community of friends, family and neighbourhood. You will work closely with other community partner organisations in order to deliver the best possible comprehensive service to consumers. Community Rehabilitation and Support Workers at Neami National function within a collaborative team approach and are supported by regular opportunities to reflect upon their practice and access comprehensive training and development.
Peer Support Workers are integral members of the team and draw on their lived experience of recovery from mental illness, to instil confidence and hope in others about the journey of recovery. You must have a lived experience of your own mental illness and a rich understanding of your own processes of recovery. You must be willing to purposefully use your own story to help others further their own understanding of recovery.
