About your new role

Northern Housing Pathways is a partnership between Neami National, Northern Area Mental Health Service (NAMHS) and Haven Home Safe. The aim of the program is support consumers being discharged from these clinical services that are homeless or at risk of homelessness, by providing access to housing or helping maintain tenancies.

The Pathways Housing Support Worker will be based ay Neami Fairfield (Wadamba Wilam) and NPU – Northern Psychiatric Unit. They will have a caseload of consumers living in transitional housing properties. They will support consumer to develop a housing plan and an exit plan to create a positive transition into long-term sustainable housing. They will also work with each consumers to plan future housing options and to identify what referrals they might need to other services to reduce their chance of homelessness in the future. They will work collaboratively with NAMHS clinicians and the Tenancy Manager at Haven Home Safe. The Pathway worker will also provide people with assistance through assessment and planning within the Northern Psych Unit (NPU) alongside the Social Work Team.

What you will bring to the role?

  • Experience in working with homelessness
  • Experience working with First Nations consumers
  • Experience or demonstrated desire to work in an acute inpatient unit
  • Experience/knowledge of working with people with a psychiatric disability in housing crisis
  • Excellent written and verbal communication skills

What the role requires

  • National Criminal History Check (Police check) – disclosable outcomes considered
  • Current Working with Children Check required before commencement of work
  • Current Drivers licence
  • Australian Working Rights
